Community Christian School has established certain admission guidelines. Please call the school office for further information. The list below is a minimum starting point.

• Admission Application and Registration Fee

• Notarized Medical Release Form

• Florida Health Forms 680 (Blue Shot record) and 3040 (Gold Physical form)

• Transcript for High School or most recent report card

• Standardized Test Scores

• I.E.P. if applicable

• Psychological Testing if applicable
